'Share your care' - connected care system in Adult Social Care

'Share your Care,' locally known as 'Connected Care', is a secure IT system for sharing information about a service user's medical conditions and needs across health and social care organisations. The system enables instant, secure access to patient and social care records across Berkshire, for professionals involved in that person’s care.

What are the benefits for the service user?

Some service users might expect that the people who support them already have access to relevant information about them, but that hasn’t been the case – until now. The aim of Connected Care is to give the service user:

  • Safer and faster treatment
  • Reduced duplication of tests and results
  • Reduced delays to treatment and care
  • The person only having to tell their story once

 How is it accessed?

Authorised users within partner organisations are provided with login details giving them access to the Connected Care data where they can look up relevant health and care records of the service user whose care they are involved in.

Why has it been created?

A number of key organisations including GPs, hospitals and social care teams can all hold important information about a person’s care, but this information is not always easily available to all relevant partners. Therefore the programme has been developed in partnership with Patient Focus Groups in East and West Berkshire.

What are the benefits for health and social care professionals?

The system enables instant, secure access to patient and social care records across Berkshire, supporting them in providing the best possible care by enabling them to make decisions based on the most up to date information about the service user.

What other partner organisations use Connected Care?

Current partners in Berkshire include: GP Practices in Berkshire, Surrey Heath and Buckinghamshire, Bracknell Forest Council and Wokingham Borough Council Adult Social Care, Royal Berkshire Hospital, Wexham Park Hospital, Heatherwood Hospital, Frimley Park Hospital and Berkshire Healthcare NHS Foundation Trust Community & Mental Health.
