Transition Nurse
We are pleased to announce that from Dec 2014 we have appointed a transition nurse from Royal Berkshire Hospitals NHS Foundation Trust. She will be based at the Royal Berkshire Hospital but will facilitate transition across the Thames Valley SCN.
Dr Arvind Nagra who is a Consultant Paediatric Nephrologist at University Hospital Southampton NHS Foundation Trust has developed the Ready Steady Go - TIER Network where the medical team work towards equiping patients with the skills to make a successful transition to adult services, covering issues such as medical knowledge, independence, health, career options and lifestyle in the three phases of the programme.
