Professional Live in Care, Supported Living and Support and Care at Home (16 to 65)
Cedar Hope Provide a range of services to adults with various needs including young adults (16 to 25 yrs old) and adults (25 to 65 yrs old) with various needs and conditions including autism, mental health, learning disabilities, epilepsy, Alzheimer’s and dementia. This CQC Regulated service is available to Private and Public Paying Clients needing Support at Home or Within Our Supported Living Houses in Reading including those discharged from hospital. Cedar Hope Care Services Ltd (part of the Independent Social Work Partnership (ISWP) Group is a Reading based company registered and regulated by the Care Quality Commission (CQC).
Cedar Hope Care Services Ltd is led by qualified professionals including registered nurses, social workers and carefully vetted support workers experienced in working within the private and public sector, community nursing teams and the social care sector.
Below is an outline of the services we offer
- Hospital Discharge :
We offer support to people discharged from hospital who need support in their homes or care and respite support within our supported living accommodation in Reading
- Respite Care and Support
We offer respite services to young adults and adults with mental health, autism, learning disabilities, epilepsy, dementia, Alzheimer’s and other care and support needs
- Live-in Care and Support
We provide the best high quality 1-1 live-in care within Berkshire area and surrounding counties. We work with the client, family members and significant others to identify and provide a carer matched with the client and the client’s needs. This service allows each client the best possible care and the option to remain in their home and familiar surroundings.
- Support and Care in the Home (Domiciliary Care)
We provide this service to people to enable them to continue living in their own homes. We can offer bespoke hours at intervals as required by the client.
- Supported Living for adults 16 to 65
We provide this service within our excellent supported living shared accommodation located in Reading, Berkshire, and nearby counties. This support is especially available to young adults aged 18 to 25 who may present with greater need for support due to conditions such as mental health problems, autism, learning disabilities and epilepsy. We can provide 1:1 or 2:1 care if required.
- Professional delivered re-enablement services
Cedar Hope, in partnership with other partners offer other services to all clients at reasonable additional cost for re-enablement support via our experts such as psychiatrists, psychologists, specialists social workers, occupational therapist and specially trained therapists who specialise in talking therapies and other forms of therapy.
